Update documentation for approval workflow and Github app

The above PR adds an approval workflow that triggers the publish-bicep workflow once approved. This is done via a Github app that has access to the necessary secrets and info to run the publish-bicep workflow. We should add documentation in this repo on the Github app and how we expect the approval+publish flow to run and add info to a TSG for debugging purposes.
