All options for push notification work?

I am 99% sure that the "survey" option doesn't work - when you select it, there is no option to specify a survey URL.
Ditto for "website".
What parts of the push notification code have been tested and are known to work?

To test push notifications, you need to have:

• an app with the push notification API key configured
• a database with the user/phone push token
• the push notification API key configured on the server

The NREL OpenPATH app has the push notification API key embedded, I can provide the server API key internally and we can use a dump of the staging database (which has the phone push tokens) to send to stage/test phones.
