The Markdown All in One plugin does not let you add a column inside the TOC is generates. Have to find a workaround

The IPT TOC is a just wrapped with a TD HTML block.

Yes, I know but the extension doesn't let you put anything in the middle of the TOC. I think we need to disable the extension and then make the changes.
