File conversions fail silently

When a file conversion fails, due to parsing or other error, the user needs to be given a notification so that they can take corrective action. Currently the an empty destination file is displayed without explanation.

In the case of syntax parsing, the user should be notified of the location of the error.
In the case of the code failing, diagnostic information should be supplied to the user so they can file a bug report.
