Issues discovered in Conway and fixed during the bootstrap phase

This is a top level ticket that lists all of the issues that have been or will be discovered right before or after the Chang hard fork. This ticket can be closed once all of these issues are resolved and we have hard forked out of the bootstrap phase on mainnet:
